How do I create a form that is conditional based on another field?

I am creating a basic form in rails where a user is able to create an opportunity. The user is given the ability to specify if they are an employer or an employee. If they are an employer the form is made very different.
So my question is, how do I create a conditional form? So I don’t have to create different forms for users.
Also, if it is possible to combine logic into a custom helper.

JPIV is a software package for simulating the flow through porous media. It is developed in order to facilitate the use of particle image velocimetry (PIV) technology for assessing the flow of a fluid in a heterogeneous media.
The program can be used to study internal and external flow in a porous medium and includes PIV algorithm and predefined boundary conditions.
Due to the complex nature of PIV technology, a number of tools have been included in the program that make it possible to visualize, model, enhance and analyze the data obtained by PIV.

System analysis: This plugin allows one to get a detailed analysis of the fluid velocity field by using a set of data points.
Although the program includes many functions, there is still a large number of features that can be incorporated or included in the future versions of the program.
